A good idea is to
create a CV, which will aid in the entry of this data.
From the Office of Samuel K. Parrish, M.D., Associate Dean for Student Affairs In order for your Dean's letter to be released for mailing to residency programs or for electronic transmission through the ERAS system to your residency programs, you will need to complete and submit a Dean's Letter Release Form to Shay Myers in the Office of Student Affairs (Queen Lane Campus). Forms can be downloaded from this web site or are available in the Office of Student Affairs and in Pittsburgh in Jeff Reed's office. Please note there are two different release forms, one for ERAS for electronic transmission of Dean's letters (Form A) and one for Non-ERAS for Dean's letters which need to be mailed (Form B). For those students who will be participating in program applications through ERAS but who also may be having some applications and Dean's Letters mailed, you will need to use both types of release forms. ERAS participants will need only Form A filled out. Students participating in programs which are not a part of Electronic Residency Application System, should attach program mailing labels to a single Non-ERAS Dean's Letter Release Form (Form B). Students who wish to add additional Non-ERAS program labels for Dean's letter mailing must fill out an additional Non-ERAS Dean's Letter Release Form (Form B) and attach the additional labels. Whether your application will be electronic through ERAS or through the mail for Non-ERAS programs, the absolute deadline for receiving your Release Form is October 1st. If our office has not received your Release Form(s) by that date, your Dean's letter mailing and or ERAS transmission may be delayed beyond November 1st.
When your Dean's Letter is complete, you will be emailed as to the time that you MAY review the Dean's Letter before they are released. Special accomodations will be made for students at the Allegheny Campus or that may be a great distance away from the Queen Lane Campus. Your Dean's Letter is CONFIDENTIAL, and a copy CANNOT be kept for your records.
